Case Manager -(DHS) Casa El Faro
Comunilife Inc. is looking for an experienced Case Managers for its new sanctuary program opening in the Bronx in January 2024,  Casa El Faro.  The Case Manager is responsible for conducting intakes, initial skills assessments, providing case  management and offering referrals for job placements, permanent housing placements or to other  appropriate social services based on client need. The program services families so case managers will be expected to work with the whole family unit, providing support for access to education, child care, after school services and other needed family support.  Provide case management services for new  immigrant and refugee clients. This position is eligible for Comunilife's generous benefits and paid time off package including medical, dental, vision, life insurance, retirement employer contributions after 2 years, tuition reimbursement and professional development opportunities.     Essential Duties:   Screens potential clients for program enrollment and orient clients to the program and describes  program objectives.  Conducts initial intake assessments. Utilizes assessment tools (CARES) to identify support  service needs and makes immediate referrals.  Provides assistance to clients in the job placement process, including taking clients to  interviews, providing job case management, helping with application completion, etc  Provides assistance to clients in the post placement case management process, including  ability to retain the job  Tracks and monitors participant progress. Oversees weekly follow up via one on one sessions.  Follow up with participants through mentoring and counseling. Meets with participants to  address issues, challenges and successes to strengthen client's capacity for long-term self sufficiency.  Maintains confidential case record for all clients.  Maintains contacts with outside agencies to refer or obtain additional client services.  Conducts collaborative efforts with service providers to maximize· service delivery.   Facilitates workshops addressing issues related to assisting with career development: skill  building, résumé drafting, understanding worker’s rights, and training if applicable.  Participates in ongoing education and professional development through courses, seminars and  other in-service training in the areas of services for the homeless, cultural diversity training,  supportive counseling and workforce.  Provides monthly service outcomes information and supporting documentation to Supervisor for reporting purposes as required by funding agencies.  Performs all other duties as requested by managerial staff and other duties as required.
