POSITION SUMMARY:

Center for Family Services is seeking an experienced and dedicated Case Manager for the Fund My Future program. Center for Family Services will deliver wraparound services to participants enrolled in the Fund My Future program with Atlantic Cape Community College. These services provide a continuum of services and direct assistance to support the success of the program participants.

The Case Manager will provide one-on-one participant coaching and will support and work with Atlantic Cape staff to ensure access to a support network, financial literacy, linkages to community resources and services, childcare, and transportation. The Case Manager will be trained in Motivational Interviewing, Trauma-Informed Care and Mental Health First Aid.  In addition, the Case Manager will provide individual assessments.

Duties and Responsibilities:

Provide case management and follow up services to program participantsDevelop program service plansProvide life skills/support focused on interpersonal skills, conflict resolution, financial planning, communication, planning and organizing, critical thinking teamwork, professionalism and more.Be familiar with internal and external support servicesIdentify and address barriers. Work closely with internal CFS staff and partners to provide seamless services to participantsOther duties as required

REQUIREMENTS: 

Minimum of a bachelor's degree in related field or relevant work experience.Demonstrated proficiency in using Microsoft Office applications or online database tracking systems. Three years' experience in a social service settingCase Management experienceAbility to maintain a leadership role with the participants to motivate and influence positive behaviors.Excellent verbal and written skillsCurrent driver's license & a good driving record of 3 years.Ability to work in multiple countiesVehicle with current insurance and registration.Available to work some weekends and evening hours.
