Case Manager-(ESSHI) El Borinquen Job Details Job Location Bronx, NY Position Type Full Time Education Level 2 Year Degree Salary Range $40,000.00 - $45,000.00 Salary Travel Percentage None Job Shift Day Job Category Nonprofit - Social Services Description Comunilife Incis looking to fill a Case Manager position for its Bronx residential program. The Case Manager develops supportive relationships with clients; link them to community- based medical and mental health care, and provide crisis intervention as needed. The Case Manager provides support with accessing primary medical care and other necessary services to increase clients’ capacity for independent living and monitors progress, providing referrals as needed. The Case Manager completes required documentation in a timely manner and participates in case conferences, on-site and community-based meetings and trainings as required. Essential duties: + Completing comprehensive assessments, service plans, progress notes + Monitoring and reassess clients’ needs for referrals and services + Provide psychoeducation on relevant topics like substance use disorders, entitlements, etc. + Provides individual and group workshops + Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented. Qualifications Education and Experience Requirement(s): + Bachelor's Degree in human services or related field; Credentialed Alcohol and Substance + Abuse Counseling (CASAC-T or CASAC) is a plus + Two (2) years' work experience + Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus + Bilingual skills in English/Spanish a plus + Knowledge and hands-on experience in providing direct services related to the treatment + and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness + Excellent oral and written communication skills; proficiency with Ms Office and Google + product suites + Ability to perform and manage in a team approach; excellent interpersonal skills + Ability to handle complex challenges and balance multiple demands on time and attention + Ability and willingness to perform all duties as workload necessitates Comunilife is an EEO employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
