Case Manager - Franklin House
18.00 per hour | Posted 25-Sep-2024 (EST) | Franklin Township, NJ, USA | Hourly | Direct Care | Part Time The Franklin House Transitional Housing Program provides housing for homeless women and their children in a home like atmosphere. The families are each provided a private and comfortable living space in which they reside for approximately 12 months. During this period of time, the women are involved in educational programs, build work-readiness skills, secure employment, gain parenting, financial and other daily living skills. Staff are on-site 24/7 to provide services and support as needed. Desired Qualities of a Case Manager: + Positive communication + Excellent supportive counseling skills + Compassion + Reliability & dedication + Desire to make a difference + Sound ethical and moral values + Professionalism + Conflict resolution Qualifications: + High school degree required, Associates or Bachelors degree preferred + Ability to pass criminal/registry background checks/drug screen + A valid driver's license, no more than 5 points or DUI in last 3 years + Ability to pass and maintain required Agency and program- specific trainings + Experience working with the homeless population preferred Essential Duties and Responsibilities of a Case Manager: + Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc. + Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing + Provide transportation as needed to assist with child care, grocery shopping and other needs in the community. + Support families personal outcomes to enhance quality of life + Utilize an electronic health record (EHR) system to write clear and concise progress notes + Ability to pass all Agency and program specific trainings Training: + All employees are required to complete 10-15 trainings upon hire and on an ongoing basis. + Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting. + All trainings are paid.
