The starting rate for this position is $27.50 The rate of pay displayed reflects the beginning of the pay scale. Job Summary: Case Managers utilize a systematic and collaborative approach to healthcare services for a defined population. The framework includes five components designed to enhance quality and cost-effective outcomes: assessment, planning, implementation, evaluation, and interaction to meet the individual patients’ health needs while decreasing the incidence of duplication of care. The Case Manager works closely with, and delegates care to, the High-Risk Social Worker /Behavioral Health Clinician after the assessment and determination of social needs. Job Requirements: + Bachelor’s degree required in healthcare related field + Active State of Maine license in designated profession + Minimum of five years of broad clinical experience preferred. + Case Management and/or Utilization Manager experience preferred. + Certification as CCM or ACMA preferred. + Proficient in Microsoft Office. + Strong verbal and written communication skills. + Strong interpersonal skills. + Ability to work within a stressful, fast-moving environment. + Knowledge of community agencies and resources preferred Covenant Health Mission Statement We are a Catholic health ministry, providing healing and care for the whole person, in service to all in our communities.
