The Case Manager I, II, or III for Children and Adolescent Mental Health Services, provides case management services for individuals with mental illness, substance use disorders and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Starting Hourly Rate:
Case Manager I: $16.73
Case Manager II: $17.56
Case Manager III: $18.44
Education/Experience Required:
High School diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required Bachelor's Degree in education, social work, psychology or human services related field is preferred. At least one year of experience in a setting working with children and adolescents is preferredAdditional compensation will be considered for any new employees who speak multiple languages
