New River Valley Community Services is seeking a Case Manager I or II at New Horizons! The Case Manager provides case management for clients of the crisis stabilization program, including clients with mental illnesses and co-occurring substance abuse. The Case Manager coordinates services, resources needed by the client during residential services, and monitors outcomes up to 30 days to post discharge. This role also provides coordination for discharge services with agency and community resources. In this role you will coordinate with Special Justices, program psychiatric staff, and Independent Evaluators for commitment hearings held on site. Starting Hourly Rate:
Case Manager I: $16.73-Depending on Experience
Case Manager II: $17.56-Depending on Experience
Education Requirements:
Case Manager I:
High School Diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required. Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is preferred. Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is preferred.Case Manager II:
Graduation from an accredited college or university with a Bachelor's Degree in Social Work, Psychology or other related field is required. Registration as a QMHP or trainee is required. Must maintain QMHP status and all required trainings/CEUs throughout employment.
