The Norfolk Community Services Board (NCSB) is the primary provider of public mental health, substance abuse, and intellectual disability services for the City of Norfolk. A continuum of services spans from outreach and prevention through intensive psychiatric care, provided by highly clinically oriented service professionals with expertise in these service areas. Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Please join us as we provide hope and recovery opportunities for the persons we serve. The NCSB is currently seeking an experienced Case Manager III to work with Child and Adolescent Services. The Case Manager III will maintain a case management caseload of children and adolescents with serious emotional disturbance (SED). SED case management services include assessment, treatment planning, linking, monitoring, and advocacy. Department Hiring Salary Range: $46.589-$53,577 Essential Functions Essential functions include but are not limited to: + Completes a face-to-face assessment to determine needs and strengths, eligibility for service, progress and to identify specific services and supports at least every 90 days. Face-to-face contact occurs through home visits as often as possible but may also occur in the office, or in another community setting. + Provides SED Case Managment services to include linking to services, resources, and supports specified in the ISP; coordinating services and service planning with other providers within NCSB and with other agencies, and private providers; and monitoring progress, services delivery, physical and mental stability, and consumer satisfaction by providing weekly service to all individuals on assigned caseload. + Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing by completing documentation and submitting filing according to program Standards. Completed an accurate record of direct and consumer related service on a daily basis. + Provides practical counseling and education to guide the individual in problem solving strategies and to develop a supportive therapeutic relationship. This may include encouraging compliance with treatment, substance abuse education, discussing relapse prevention strategies, stress management, etc. Performs crisis intervention to resolve critical situations. + Assist consumer/family in obtaining needed services, i.e. hospitalization, housing, food, medications, etc. This may involve ensuring transportation for consumers who have no other means to get to appointments quickly. + Comply with regulations for HIPPA, mandated reporting of abuse and neglect, consumer Confidentiality, and other Consumer Rights by reporting any suspected violation of those rights and regulations to Supervisor and Child Proctective Services. + Update clinical data as necessary for consumers on assigned caseload. Education/Experience Bachelor's degree in a human services field. Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations. Preferred Education/Experience: + Master's degree with considerable experience (2 years minimum) working with children and adolescents in the mental health field. If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit.
