Please read below and apply with an updated resume for consideration: **Type:** Three - month contract, opportunity for extension or fulltime offer **Schedule:** Monday through Friday, 8am to 5pm **Pay:** $32.85 - $34.85 per hour **Responsibilities of the Case Manager LPN:** · In this role, you assess the environment, functional and psychosocial status, and financial situation of a patient, consult their physician, and develop a program or action plan to empower them as they go through care.
