About the Role: Amergis is hiring for a leading North Bay mental health services facility seeking a passionate Case Manager to empower clients towards independent community living.
Key Qualifications (HARD REQUIREMENTS):
+ Educational Requirement:
+ Associate's Degree with 6+ years mental health experience, OR
+ Bachelor's Degree with 4+ years mental health experience, OR
+ Master's Degree with 2+ years mental health experience
Responsibilities:
+ Develop personalized client service plans supporting recovery and independence
+ Provide comprehensive case management including:
+ Psychiatric rehabilitation
+ Symptom management coaching
+ Independent living skills development
+ Health and wellness education
+ Housing and budgeting assistance
+ Navigate complex healthcare systems
+ Facilitate community integration and peer support connections
+ Conduct medication management and adherence support
+ Provide crisis intervention services
+ Document clinical records and agency performance metrics
Essential Skills:
+ Strong communication across team and community providers
+ Cultural sensitivity
+ Crisis management capabilities
+ Ability to teach daily living skills
+ Proficient in MS Office
+ Valid California driver's license
