Overview

We are looking for a Case Manager PC to join our team. This position will directly report to the Branch Director PC or Assistant Branch Director PC and is responsible for working collaboratively to assess, plan, facilitate and advocate for the options and services to meet a patient’s health needs. 

Coordinates with the Case Manager Supervisor and with DADS to complete the service plan authorization processCoordinates all aspects of service delivery, ensuring that all services are delivered according to the service planCompletes on-going monitoring with the client.
