Job Overview:
The RN Case Manager works collaboratively with the interprofessional team, including physicians, social workers, clinical nurses, home care services and others as needed. This role actively participates in specific clinical initiatives focused on reducing the length of stay and continued stay denials, improves efficiency, quality, and resource utilization.

The Case Manager RN works collaboratively with patients, physicians, family members and healthcare providers to implement a plan of care which meets the individual s needs. This is coordinated by assessing, planning, implementing, advocating, monitoring, and evaluating options and services, to meet individuals healthcare needs.
