Job Description

The Case Management nurse is skilled in blending multiple functions related to utilization management, risk management, performance management and discharge planning. The case manager is in a proactive, patient centered approach to maintain quality and enhance resource usage. The Case Management nurse promotes collaborative efforts with all disciplines to promote patient welfare, resource management and quality of care. The Case Management nurse must be versed in both governmental and large group health plans requirements and regulations.

 

Education / License / Certification:

Currently licensed registered nurse in the state of Tennessee or compact state in good standingBSN degree preferredCase Management experience preferredCCM certification preferred

Experience / Skills:

Three (3) years of acute care clinical experience, intensive care nursing desiredStrong interpersonal/communication skillsSelf motivated, can work autonomously and ability to work with an interdisciplinary teamComputer and statistical experience highly desiredMust be able to work in a fast paced, ever changing environmentAttention to detail and concentration
