Case Manager RN - Per Diem
Job Summary The per diem Case Manager will have a strong knowledge of the case management process and standards of practice for Case management. responsible to assist in the assessment, development, planning, and coordination of care. Assumes a leadership role within the interdisciplinary team to coordinate services through the continuum of care to achieve optimal clinical and resource outcomes. Essential Functions • Flexible, able to assume caseloads with minimal assistance. • Able to relate and work with chronically ill and disabled patients of all ages, and with emotionally upset, and at times hostile people within the faculty. • Accesses legal assistance for guardianships or power of attorney as well as protective services for elderly and children. • Acts as a resource for staff, families, patients and physicians. • Completes initial D/C planning assessment. • Able to relate and work with various managed care representatives, defining the continuing need for medical treatment. • Appropriately documents the discharge plan, confirming commitment of the referral source, including all communication and referral data. • Attends the daily Discharge Meeting and the IDT rounds and documents information in EMR. • Refers cases to Director of Case Management and /or physician advisor as needed. • Assumes Director's role of case management in their absence. • Coordinates with all members of the treatment team, patient, and family to assure coordination of care, decision review, fiscal communications, and discharge planning by collecting and assessing data. • Communicates with appropriate agencies for authorizations and referrals. • Differentiates culturally specific care into discharge plan and identifies communication and learning barriers for all populations which may interfere with continuum of care. • Refers patients with after-discharge needs to appropriate Case Manager and keeps her/him apprised of anticipated patient discharge dates and needs. • Provides assistance with indigent and self-pay population to appropriate disciplines. • Monitors length of stay and decreases where appropriate. Actively works to decrease cost per case. • Monitors physician progress notes to ensure that adequate and timely documentation exists to support medical necessity and appropriateness of continued hospital stay. • Reports variances according to established care quality improvement process. • All other duties as assigned. #STC Qualifications Education Requirements 4-year bachelor's degree - OR - combination of relevant education & experience may be considered in lieu of bachelor's degree Completes and maintains all competencies and trainings as required for role.
