**This is a remote position, but frequent travel may be required to visit members at home, hospitals, skilled nursing facilities, or physician offices.** We are seeking an experienced Registered Nurse Case Manager (RNCM) to join our Senior Care Options (SCO) team. In this role, you will manage the care of frail elderly members, working closely with physicians, caregivers, and community resources to ensure a comprehensive, patient-centered approach. **Key Responsibilities:** + Conduct initial and ongoing geriatric assessments + Develop and communicate individualized care plans + Complete Minimum Data Set-Home Care (MDS-HC) assessments + Coordinate community resources such as Adult Day Health, Adult Foster Care, and Personal Care Attendant Programs + Participate in primary care team meetings + Manage transitions of care across various healthcare settings (ED, hospital, rehab, nursing home, home care) + Serve as an advocate and facilitator to resolve barriers to care + Provide education and coaching to members and caregivers on health status, treatment options, and benefits **Qualifications:** **Skills:** + Strong ability to work autonomously and prioritize workload + Excellent team collaboration and problem-solving skills + Experience navigating community resources and care coordination + Cultural competency and sensitivity to diverse populations **Education:** + BSN degree and Registered Nurse (RN) license (unrestricted, active) **Experience:** + 3+ years of clinical nursing experience + Case management experience managing geriatric/chronic illness populations + Experience in Senior Care Options (SCO) programs preferred + Knowledge of Medicare/Medicaid managed care preferred + Proficient in health information technology and computer systems + Case Management Certification (CCM) a plus **About US Tech Solutions:** US Tech Solutions is a global staff augmentation firm providing a wide range of talent on-demand and total workforce solutions.
