Position Summary 

Manages patient's transition through the healthcare system and transfers accountability to the appropriate person or agency upon discharge. Performs admission, concurrent and retrospective review for all patients using Interqual Criteria. Provides necessary information to 3rd party payers, physicians, staff, patients and families. Accountable for accurate resource utilization and facilitation of improvement in quality care for assigned caseload. Provides services to patients throughout the lifespan including all age groups.


Minimum Qualifications 

Minimum Education: Graduate of a Social Work BSW or MSW program.Minimum Experience: Must possess familiarity with case management issues, discharge planning and utilization management.Licensure/Certification: Must possess a current New Mexico license as a Social Worker.  Interqual Certification or Case Management Certification preferred.

Certification or completion courses must meet the standards of Memorial Medical Center.
