Case Manager (SSG) Job Details Job Location Pikeville - 173 Redale - Pikeville, KY Position Type Full Time Education Level High School Salary Range $19.50 - $19.50 Hourly Job Shift Day Description Position Summary:The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.Essential Job Functions:• Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery;• Assesses the needs, conditions, and desires of assigned program participants;• Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc.• Assists participants with establishing or re-establishing community support systems;• Acts as advocate on behalf of program participants to improve access to vital services;• Participates in clinical staffing decisions regarding the participant’s current strengths and resources, and their need for additional resources;• Completes documentation of services provided in a timely manner;• Provides input into the patient’s treatment needs and disposition;• Conducts assessments of program participants and provides therapy as needed;• Reviews and documents all clinical services and activities;• Participates in case conferences involving assigned program participants;• Prepares Discharge Summaries, as needed;• Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community;• Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation;• Performs any other duties as assigned. Qualifications Essential Qualifications:Certifications/Licensures:• Veterans are perferred• CADC or similar certification preferred; Preference will be given certified candidates;• Valid driver’s license and the ability to obtain and maintain WestCare’s vehicle insurance is required.Education:• Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;• High School Diploma or equivalent is required;Experience and Competencies:• Experience working with individuals suffering from mental health and substance abuse disorders required;• At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;• Ability to define problems, collect data, establish facts, and draw valid conclusions required;• Excellent communication skills are required;• Ability to exercise good judgment and discretion required;• Ability to work well in a team environment required ;• Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;• Professional appearance and demeanor required;• Must be culturally/linguistically sensitive to populations served;• Intake and assessment experience is highly preferred;• Bilingual ability is preferred;• Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
