EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join the Stop Lift/ Anger Management program.

Schedule: Monday-Friday 9AM-5PM
Location: Hempstead, NY
Reports to: Program Supervisor
What drives the core purpose of this job:
The Case Manager is responsible for coordinating all aspects of client intake, scheduling, case follow-up, and administrative support for the STOPLIFT/AMP program. This role requires strong organizational and communication skills to liaise with clients, courts, probation departments, and other referring agencies. The Case Manager may facilitate structured courses for first-time offenders, maintain program records, and ensure compliance with reporting requirements.
What are the key Responsibilities:
+ Client Coordination & Registration:
+ Coordinate all aspects of client intake, scheduling, and case follow-up.
+ Register clients for the STOPLIFT/AMP program via telephone or on-site.
+ Schedule and set up classes for adult and juvenile clients.
+ Program Administration & Documentation:
+ Prepare and maintain client information, program correspondence, and STOPLIFT/AMP reports for referring agencies.
+ Maintain organized STOPLIFT/AMP program files.
+ Prepare and submit monthly and quarterly case statistics.
+ Complete all necessary paperwork, including attendance sheets and class lists on class days.
+ Facilitation & Client Support:
+ Facilitate classes for first-time offenders charged with shoplifting, assault, harassment, and similar offenses, following the structured curriculum provided by EAC NETWORK.
+ Assist supervision and staff with office work, including correspondence and reporting to referring agencies.
+ Communication & Liaison Duties:
+ Handle telephone calls and contacts with the probation department, courts, and other agencies, providing updates to the Program Director.
+ Answer, screen, and direct telephone calls appropriately.
+ Assist coordinators with scheduling classes and organizing class paperwork for adult and juvenile clients.
+ Administrative & Office Support:
+ Order and maintain inventory for program office supplies.
+ Monitor intern and staff attendance.
+ Generate purchase orders for the program.
What key knowledge, skills, and abilities are required to excel in this role:
+ High School Diploma or educational equivalent required. Degree in Social Work, Criminal Justice, or a related field preferred.
+ Prior experience in case management, program coordination, or administrative support in a human services setting.
+ Strong organizational, time management, and communication skills.
+ Ability to work collaboratively with diverse populations, including clients, court personnel, and social service agencies.
+ Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and database management.
+ Ability to handle confidential information with professionalism and discretion.
+ Experience facilitating structured educational or behavioral intervention programs is a plus.
