As a Case Manager in our Substance Use Disorder (SUD) Program, you’ll provide life-changing support to those transitioning back into the community after incarceration. Salary: $26-27/ hour at 35 hours per week Schedule:2-3 days in the Riverhead or Yaphank correctional facilities from 7:30-3:30PM. Remaining days will be in office from 9:00AM-5:00PM Location: Central Islip, NY
What You’ll Do:
Work directly with individuals who are preparing to re-enter society from Suffolk County correctional facilities, offering crucial support in substance use recovery and transition planning. Collaborate with correctional facility staff and community organizations to develop customized reentry plans that connect clients to post-release services, such as healthcare, housing, and vocational support. Facilitate access to essential services like Medicaid, benefits, and Medication Assisted Treatment (MAT) providers, ensuring smooth transitions and reducing the risk of relapse. Act as a trusted guide for individuals re-entering the community, providing ongoing care management and connecting them with peer support networks. Track client progress, maintain accurate records, and contribute to a data-driven approach to improve outcomes for those in need. Enjoy a flexible work schedule with time split between correctional facilities in Riverhead/Yaphank and our Central Islip office.
What We Are Looking for?
A Bachelor’s Degree in Social Work, Mental Health Counseling, Criminal Justice, or CASAC, with at least two years of experience working with substance use or criminal justice-involved populations. Passion for helping vulnerable individuals overcome challenges and succeed in their recovery journey. Strong communication and collaboration skills, with the ability to work with a wide network of community and healthcare services. Proficiency with digital tools and systems, including Microsoft Office and client databases. A valid NYS driver’s license, personal vehicle, and insurance to help transport clients to appointments when needed.
