Case Manager Supervisor

Position Summary:

The Case Manager Supervisor provides leadership and oversight to a team of Case Managers responsible for ensuring the safety, well-being, and permanency of children and families involved in the child welfare system. This role ensures compliance with the Florida Child Welfare Practice Model, guiding case managers in delivering trauma-informed, community- informed, and family-centered services.

The Case Manager Supervisor plays a key role in achieving program goals, including reunification, placement stability, adoption, and permanency planning while upholding best practices in child welfare.

 

Essential Job Functions:

Provide guidance and oversight to the case management team to ensure high-quality service delivery. Monitor and evaluate case management staff performance, providing coaching and professional development opportunities. Participate in and lead case conferences, team meetings, and training sessions. Ensure staff adherence to HIPAA, Professional Ethics, Consumer Rights, and Confidentiality policies. Participate in performance improvement activities and uphold the mission, vision, and guiding principles of WestCare Foundation. Conduct needs assessments and develop case plans in collaboration with child and families. Oversee monthly case plan reviews to track progress and make necessary adjustments. Conduct individual case management sessions and provide direct support when needed. Serve as a liaison between clients, families, and external service providers to ensure coordinated care. Assist with arranging and coordinating of transportation services for clients, as necessary. Develop, implement, and monitor case plans to achieve permanency for children within 12 months. Work closely with legal services, placement units, and behavioral health providers to ensure comprehensive support for children and families. Provide safety management services, including crisis intervention and coordination of resources in alignment with in-home safety plans. Support case manager in establishing and maintaining meaningful connections for youth with kin, fictive kin, and mentors to promote life-long relationships. Facilitate reunification efforts while maintaining concurrent planning for alternative permanency options. Minimize the length of time children wait for adoption by expediting the placement process and finalizing adoptions, particularly for children with special needs. Partner with local school liaisons to increase stakeholder involvement for the educational needs of children, supporting individualized education plans and academic success. Support pre-independent living programs by developing and implementing life skills education plans for older youth in care. Promote placement stability by providing guidance and support to foster and adoptive families. Ensure accurate and timely documentation of services in compliance with CCWIS and other regulatory requirements. Support and assist in the preparation of Judicial Review Social Study Report(JRSSR) and other reports for courts, probation officers, and other involved parties. Oversee discharge planning in coordination with treatment teams to support successful transitions. Ability to work flexible hours to include some nights and weekends. Perform other duties as assigned.
