Case Manager(TEMP)

POSITION SUMMARY:

Person in this position will be primarily responsible for the case management of a caseload of individuals with disabilities that are experiencing literal homelessness or are at imminent risk of becoming homeless. The case manager will work closely with the disability advocate while providing rapid rehousing services. Essential Job Functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

Serve as an active part of the Housing Disability Advocacy Program (HDAP) team and carry out all required responsibilities as needed. Intake and assessment of incoming participants to determine eligibility and appropriate services. Manage of a caseload of homeless participants including maintenance of client files, development of services plans, and tracking pertinent data. Make referrals and appointments with appropriate agency, services including but not limited to health care providers, employment offices, substance and mental health treatment programs, public assistance, and other mainstream services. Assist clients in locating safe and stable housing. Build and maintain relationships with property managers and owners. Maintain client files according to company policy and contract requirements. Heavy documentation; Homeless Management Information System and WestCare’s Clinical Data System. Provide daily, weekly, and monthly reports as requested. Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested. Develop and maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested. Provide follow up after housing to insure client stability and successful outcomes. Attend scheduled meetings and presentations with referral agencies Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform all other duties as assigned.
