Case Manager - Young Adult Program

The Case Manager serves as the primary transition facilitator by providing the coordination of treatment, treatment planning, and the development and implementation of individualized service plans for youth and young adults with mental health needs. The case manager facilitates services – like school, medical providers, family visits, etc. – and also assists in skill building through coaching and one-on-one work with young adults ages 16-21.

ESSENTIAL DUTIES & RESPONSIBILITIES

Works with the transition team, including the member, their therapist, residential staff, guardian(s) and others to develop and implement a Thresholds Care Plan. Serves as the primary contact with the members’ support network to assist with the members’ goals for transitioning to adulthood. Provides direct service to members through assessment, case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations. Performs and participates in crisis intervention in accordance to Therapeutic Crisis intervention (TCI) and Thresholds standards. Models best practices for TCI to other staff. Performs and supervises physical management when necessary. Attends all court appearances, administrative case reviews, DCFS meetings, and Thresholds meetings as assigned. Assures that all Thresholds procedures regarding the handling of member’s funds are followed and that all safeguards, reports, and accounting meets the highest standards. Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds, DCFS, ICG, DMH, courts, and others regularly and when required. Takes DCFS CWEL training and passes the exam, if required by program.

MARGINAL DUTIES & RESPONSIBILITIES

May be required to work flexible schedules and be available to work on weekends or evenings May be required to be on call.

REQUIREMENTS

At least 21 years of age Bachelor's degree in Social Work or related field required, Master's degree preferred At least 2 years of previous experience working with youth with mental health needs, in a residential and/or mental health setting A valid driver’s license, current car insurance, and daily access to your own car Willingness to provide transportation to clients To be able to perform Therapeutic Crisis Intervention therapeutic restraint techniques, which require moderate exertion, being able to lift 40 pounds, and the mobility to lower self to a kneeling position on the floor. This training is provided and required by Thresholds
