Case Manager - Youth Care Transition

For a Case Manager who comes at difficult problems in new ways, Nexus-FACTS has a unique opportunity to be part of a new, one-of-a-kind treatment program.

We are adding a HYBRID REMOTE Case Manager to our grant-funded Youth Care Transition (YCT) program! The YCT program is a unique opportunity to address the needs of youth with high levels of mental health and behavioral challenges who have completed their treatment in a Psychiatric Residential Treatment Facility (PRTF) or a Child and Adolescent Behavioral Health hospital (CABHH) when a discharge placement has not been located.  Our Case Manager will develop resource networks in the metro area of Minnesota and complete individualized assessments of needs for each youth and create a service plan for ongoing service that will support the youth and family both as the YCT is providing in-home services and to support the family as the YCT gradually transitions out.

Primary responsibilities:

Client Consultation & Treatment Planning

Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge Provides consultation and support to families, youth, and adults in need of services Work closely with the teams to create stabilization plans, discharge plans, and supported engagement Participates in the development of treatment planning Provides assistance to families regarding resources available to the families and youth that will support the
healthy functioning of the family unit.; serves as a resource to families for consultation and crisis intervention

Service Delivery

Works effectively with outside organizations to successfully implement plans for adults and children entering crisis services Works to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients Participate in the transition of aftercare plans Maintains and completes paperwork and compiles documentation required by the organization and for the governing/accrediting agencies; maintain current files on all individuals enrolled in services Produces informal and formal reports as requested by outside agencies, parents, referral agencies, courts, etc. Participates in discharge planning with team

Outside Organization Collaboration & Support

Communicates with families, legal systems, schools, mental health providers, probation, and social service agencies; works effectively with outside organizations to successfully implement plans for adults and children Work to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs Maintains database of community resources and contacts for outside providers and organizations; collects data on services, linkages and additional outcomes as identified

Policy, Procedure, Compliance & Training

Ensures confidentiality of all treatment and services information pertaining to all individuals and their families Participates in staff and supervision meetings and required in-service meetings Exhibits clear, professional, respectful, and effective communication when interfacing with clients, coworkers, families, and external contacts Interacts with and appreciate people from diverse cultural, social, economic, and religious backgrounds Maintain a schedule that is adjustable and flexible to meet changing work needs and demands of clients and their families; leverages organizational skills and tools to proactively manage multiple client needs/demands Represents the organization positively within the community

Required Education and Licensure:

Required to meet MN Mental Health Practitioner definition as defined in the MN Statutes/section-245.462/subdivision 17 Bachelor’s Degree is required. Preferably in Human Services Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements

Preferred Education and Experience:

Degree in Human Services 2+ years of experience managing a caseload related to youth/families Knowledge in the realm of Social Services in metro and SE Minnesota regions

Location and pay:

Hybrid remote in the twin cities, MN area $45,000-50,000 per year, full-time, salary exempt position

Travel:
Use of personal vehicle for regular local business travel will be required to support the clients and agency business offices or company sponsored events may be required.

Expected Hours of Work:

Days and hours of work are Monday through Friday, with the expectation to work specific schedules and hours as identified and communicated by management.
