*Description* *Attention Case Manager Social Worker -- Sharon Hospital has a rare opportunity!* ** *$25,000 SIGN ON BONUS* ** *Full Time Day Shift Monday - Friday 8am- 430pm, rotating weekend* ** *Purpose: *Responsible for providing social work services and discharge planning functions for the hospital. The social worker provides assessment, evaluation, education, resources as well as placement for both inpatient and outpatient for a variety of psychosocial needs, including substance abuse and mental health. Population served based on scope of services in the department which includes all age groups.* * ** *Essential Responsibilities * * Performs psychosocial assessments to determine social service and discharge/transition planning needs. * Provides support, counseling and crisis intervention. * Communicates and collaborates with health care team, as well as patient, family and other caretakers to optimize support and discharge/transition planning needs. * Involves patient, family and caretakers in the formation and revisions of the plan of care, taking into account customer preferences and assessment changes. * Maintains current knowledge of available resources and resource materials within the community. * Demonstrates the ability to be flexible, organized and function under stressful situations. * Actively participates in department staff meetings, huddles, and palliative care meetings. * Fulfills regulatory requirements, i.e. COP, TJC, and DPH. * Documentation meets current standards and policies. * Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor). * Demonstrates regular, reliable and predictable attendance. * Performs other duties as required. ** *Education and Experience Requirements: * * Master Social Worker (MSW). * Licensed Master Social Worker (LMSW) or able to get within 1 year of hire * 2 (two) years of experience in community settings, rehabilitation, or nursing home environments. *Minimum Knowledge, Skills and Abilities Requirements:* * Knowledge of Microsoft Office, including Word, Excel, PowerPoint and Outlook. * Excellent customer service skills. * Ability to multitask, make decisions and communicate effectively. ** ** *License, Registration, or Certification Requirements:* Company: Sharon Hospital Org Unit: 1176 Department: Care Coordination Exempt: Yes Salary Range: $31.25 - $58.05 Hourly We are an equal opportunity employer Qualified applicants are considered for positions and are evaluated without regard to mental or physical disability, race, color, religion, gender, national origin, age, genetic information, military or veteran status, sexual orientation, marital status or any other classification protected under applicable Federal, State or Local law.
