The University of Miami/UHealth OBGYN Department of has an exciting opportunity for a full-time Case Worker 2 (H) to work in Miami, FL. The Case Worker 2 (H) monitors outcomes, data collection, and record keeping, and coordinates services for the respective population, including providing information about professional development opportunities, quality improvement strategies, and counseling to improve the client’s social and psychological conditions. The Case Worker 2 (H) assists those experiencing difficult situations by helping them find the community resources/services that they need contributes to the development of family support plans and monitors its implementation and modification.
CORE JOB FUNCTIONS
Conducts situational analysis, which may be at home and/or office visits, mentors, and provides technical assistance as needed.
Collaborates with the service agencies and program leadership to provide and administer services appropriately.
Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
Maintains and reviews records and files to ensure compliance with project guidelines.
Collaborates with all funding sources to ensure the availability of adequate resources.
Maintains current knowledge of services available and changes applicable rules and regulations.
Attends meetings and training, which may include the Early Learning Coalition, United Way, and the Quality Counts Career Center to enhance developmental potential.
Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
The Department of Obstetrics & Gynecology is looking for an energetic and organized case worker to support women obtain the federal, state, and other agency-services and resources needed for their situational needs.
Collects and reviews care plans for input about client needs.
Interviews clients for situational needs. This might include visiting clients during an admission.
Supports and assists in the administrative tasks required in applying for identified services.
Maintain relationships with patient referral agencies and collaboration.
Collaborates effectively with agencies to administer services to clients.
Plans regular follow-up with clients to assess progress and additional needs.
Reviewing and modifying plans as circumstances change
Schedule and confirm patient appointments.
Schedule transportation for clients as needed.
Navigate clients to appointments.
Administrative and Team Assignments:
Create a weekly schedule outlining upcoming appointments and regularly update it with the outcomes of each appointment.
Enter weekly appointment and outcomes in database.
Prepare and maintain accurate and thorough case records.
Coordinate with and demonstrate a working knowledge of community resources.
Meets regularly with inter-disciplinary team.

CORE QUALIFICATIONS
Bachelor’s degree in relevant field
Minimum 3 year of relevant experience
Skill in completing assignments accurately and with attention to detail.
Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
Ability to process and handle confidential information with discretion.
Ability to work evenings, nights, and weekends as necessary.
