HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program is dedicated to providing 38 unaccompanied minors, ages 5-12, with a supportive, nurturing environment through transitional foster homes, daily education Monday - Friday, and supportive wrap-around services in the East Brooklyn section of New York City. Our program is rooted in ensuring that service delivery is trauma-informed, culturally appropriate, and tailored to the needs of each child while maintaining all program requirements of ORR.
Nuevo Camino is searching for Case Worker who provides oversight and support to these young persons. The major responsibilities of the Case Worker are to identify appropriate parents or guardians to whom the young persons may be released and to identify any concerns found with potential sponsors with the Federal Field Specialist. The Case Worker is also responsible for ensuring that the young person in the transitional home is receiving essential services such as medical, mental health, and dental care; basic education; orientation; and legal screening.
Primary Responsibilities and Essential Functions of Position:
Assess the needs of and provide support and case management services to all young persons in the program; Screen for human trafficking concerns.Ensure that individual service plans are implemented for all children in care, in accordance with all applicable regulations and policies.
Document the provision of services in each unaccompanied minor’s case file.
Arrange for medical services, health screening, and mental health assessment of young persons.
Help young people identify and screen prospective sponsors;
Assist potential sponsors with the completion of family reunification packets and refer them to fingerprinting.
Record in full detail all pertinent information in applicable databases and logs and record all movements in the database and/or unit log.
Pick up young persons from the airport or other transportation hubs as necessary and bring them the transitional home.
Adhere to program guidelines and schedules.
Document and report all major and minor incidents in accordance with agency policies and procedures.
Facilitate the timely release or discharge of the unaccompanied minor when the time arrives.
Will participate in agency meetings and training as required
