CASEWORKER

Provide casework services to children with Intellectual or Developmental Disabilities and their families, specifically:

Develop and implement service plans as part of the program multi-disciplinary Treatment Team with a focus on permanency planning and discharge planning. Provide on-going assessment of risk and safety for each individual on your caseload, making necessary referrals for children and families Develop and maintain electronic and paper case records. Ensure timely and thorough documentation in case records including Family Assessment and Service Plans, progress notes, and other reports as needed. Provide crisis-intervention when necessary Act as a liaison between ACS, DSS, OCFS, OPWDD or any other contract agency, school or service providers. Meet all city and state requirements for face-to-face contact with children and their families. Conduct home assessments and other fieldwork as required. Represent agency at court hearings, including the preparation of court reports and follow-up of court orders. Attending all necessary workshops and training required by the agency. Prepare Psychosocial Summaries within 30 days of child’s admission to the residence, and update annually or as needed. Serve on the Interdisciplinary Treatment Team; attend Team Meetings, Medication Reviews and Family Team Conferences. Serve as Team liaison to the school Committee on Special Education, attend IEP conferences and contact school personnel regularly. Provide and arrange visitation opportunities for children, parents and siblings on a scheduled and routine basis, and maintain documentation of such visits. Ensure proper Intake and Discharge procedures.

Qualifications 

Possess a desire to positively influence the lives of others. Strong advocacy, communication and organizational skills are necessary. Bachelor’s degree required in social work or a related field, master's in social work strongly preferred; other disciplines considered. Foster care experience is preferred. Ability to work flexible hours, including occasional weekend and evening hours Experience with the Intellectual or Developmentally Disabled population is preferred. Bi-lingual ability is a plus (Cantonese, Mandarin or Spanish) A Valid NYS Driver’s License is required.
