Overview The Outpatient Rehab Central Patient Services Coordinator is responsible for assisting patients and providers in scheduling and registering outpatient therapy appointments for multiple teams and specialty areas at multiple locations. FTE: Casual Schedule: Ideal candidate can work up to 40 hours per week. Variable schedule between 7:30am - 6:00pm Training will be in-person anywhere from two days to two weeks, then will move to remote work M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs. Rehab’s continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year. Responsibilities Job Description Performs appropriate registration functions, including initial entry of demographic information, insurance information, referring physician data and referral record creation. Schedules therapy appointments for multiple sites and subject areas Produces efficient and accurate schedules and records and updates schedule changes accurately Communicate changes to all involved staff in a timely manner Offers and sends confirmations for patient appointments Coordinates scheduling with outside vendors Radiology and durable medical equipment companies Provides educational information to patients regarding therapy and specialty programs including Cancer rehab, vestibular rehab, cardiac rehab, pulmonary rehab, lymphedema, video-swallow evaluations, breast wellness, wheelchair evaluations, weight management, pediatrics, feeding clinic Physician orders Reviews and verifies all physician orders for complete order requirements Requests and obtains physician orders when an order received is not in compliance/complete Tracks non-compliant orders to ensure completed prior to evaluation date Requests and obtains medical records for safe patient care when necessary Obtains and verifies physician orders and medical records as needed. Maintains working knowledge of multiple specialty services and therapy fields, clinic locations, directions, and parking information. Provides directions to patients for up to 20+ sites along with landmarks for directions and department hours and locations Provides parking information for all sites Responsible for answering multiple telephone lines and responding timely and accurately to the caller’s needs. Participates actively in department problem solving and team meetings Requests guidance and approval as needed.
