**Overview** We have an exciting opportunity to be part of our collaborative rehab team in Edina! Southdale Place offers multiple, outpatient rehab services. This OT will join a team of phenomenal lymphedema therapists and have the opportunity to contribute to improving lymphedema rehab care. In collaboration with patients and families, the therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. + FTE 0.01, casual position. + Schedule: We are looking for regular coverage on Wednesdays, with potential for growing into full-time in 2025! + Opportunity to pick up shifts at other sites too as available! Captures and applies pertinent data, performs examination, and resolves treatment diagnosis **Develops an individualized plan of care through coordination** Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals Implements and modifies care plan based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan **Partners with care team to provide skilled intervention** Implements and alters treatment to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education **Documentation/Billing** Uses acceptable medical terminology and follows professional practice standards. Completes documentation in a timely manner per department standards Stays in sync with department billing standards **Professional Expectations** Prioritizes tasks and patient care for effective time management.
