Overview We are seeking a neuro physical therapist to join our rehab team at Southdale Place in Edina! In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. FTE 0.01, casual position. Opportunity to pick up additional shifts as available and ideal candidate would have capacity to increase FTE dependent on clinic needs! Responsibilities Job Description The Physical Therapist partners with the patient and family while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and resolves treatment diagnosis. Develops an individualized plan of care through coordination Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals Implements and modifies care plan based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan Partners with patients and families to provide skilled intervention Implements and alters treatment to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education Documentation/Billing Uses acceptable medical terminology and follows professional practice standards. Completes documentation in a timely manner per department standards Stays in sync with department billing standards Professional Expectations Prioritizes tasks and patient care for effective time management.
