GENERAL PURPOSE OF THE JOB

The Centralized Medication Reconciliation Nurse (CMRN) role is a clinical registered nurse to assist in management of Home Medication entry as part of data migration between EMR systems on behalf of facility teams and serving as the clinical subject matter expert for Home Medications. The CMRN provides professional nursing care for ESKD patients specific to Home Medication management under the direction of and cooperation with the Medical Director and/or Nephrologist and in accordance with physician orders, DaVita Policies, Procedures, and Protocols, CMS, KDOQI, federal, state and local regulations for select facilities.

ESSENTIAL DUTIES & RESPONSIBILITIES

Provide Facility Support:

Extract home medication reports from historical EMR and entering home medications to new EMR systemUtilize nursing judgement to determine if medications orders meet requirements for entry into the new EMR systemCommunicate with field RN on clarifications and next stepsParticipate in feedback of EMR functionalist related to home medications in a timely mannerIdentify the need for areas of improvement and help provide resources to field teammatesAssist with the development of clinical job aids regarding clinical systems

Maintain proactive communication with facility teammates regarding patient specific medications related to the Home Medication Management Program

Train, coaching and mentor new teammates on basics of Home Medication management

Other duties as assigned.

EDUCATION AND EXPERIENCE

Required

0-2 years’ relevant experience

Vocational/Technical School Certificate/Degree

Associate’s Degree or Apprenticeship

Diploma in Nursing

Proficient in Microsoft Suite

Functional proficiency with DaVita applications

Preferred

Registered Nurse (RN) with at least 12 months of specialized renal experience

Current State licensure

TIME AND TRAVEL REQUIRMENTS

No travel required

Nights/weekends as needed

Language Skills

Fluent in the written and verbal skills necessary to successfully perform the essential functions of the position.

Work Environment

Level III – Moderate noise (e.g., business office with computers or printers, light traffic).  No required tasks involved greater exposure to blood, body fluids or tissues than would be encountered by a visitor.  The normal work routine involves no exposure to blood, body fluids or other hazardous conditions.

Visual Requirements

Vision adequate to perform the essential duties/responsibilities of the position.  May be required to demonstrate the ability to differentiate colors on a test strip.

Physical Demand

Physical requirements can vary.  These must be reviewed with management.  However, in general, the position requires the following activities:

Ability to lift a minimum of 5 lbs to a max of 50 lbs unassisted and able to stand, sit stoop, walk, stretch, reach, and use full range of body motions.Manual dexterity as required for specific equipment and ability to use computer keyboard and peripherals.

Salary/ Wage Range

$25.00 - $36.00 / hour
