Certified Diabetes Educator (Maple Grove) - Part Time

Why M Physicians?

The Certified Diabetes Educator is responsible for the education of patients, their families and appropriate support systems about diabetes self-management and related issues. The CDES provides diabetes self-management education (DSME) and coordinates protocol-based diagnostic testing and treatment, which may include medication management, nutrition, physical activity, healthy coping, risk reduction, monitoring and problem-solving. DSME is provided most often in a group setting. The CDES is responsible to ensure the program is meeting the criteria to maintain ADA recognition. This role also serves as a resource to other staff members as needed.

Certified Diabetes EducatorOverview

Key responsibilities include but are not limited to:

Provides direct patient education in an individual or group settingFollowing specific curriculum developed for diabetes educationAssesses patient's physical, psychosocial, behavioral and educational needs (to include self-management, SBGM, nutrition, physical activity, medication taking, and health maintenance labs) and interprets the information to identify individualized clinical and educational needsTeaches technical stills, concepts of diabetes self management and facilitates behavior change for patients with Type 1, Type 2, and gestational diabetes as well as those with pre-diabetesDocumentation of required assessments, interventions, evaluations, and follow-up is consistent with established departmental and ADA standardsIndividualizes education presentation & materials to the patient and patient circumstanceCare coordination is goal/outcome focused and revised and/or patient is referred for additional services to help in attaining their goalsPatient response is documented including rationale for all actions/decisionsMaintains program content so that is current and consistent with ADA standards and Fairview/M Physicians Diabetes CareUses knowledge of the organization to meet immediate and possible critical needs of patients to be seen or contacted by a providerWorks collaboratively with the other members of the health care team; including physicians, pharmacists, hospital staff and clinic staffDemonstrates accountability for professional practice by accepting responsibility for assessment of performance and peer reviewEncourages and supports the quality clinical initiatives related to diabetes careAware of the measures and monitors that are in placeStrives to help patients meet the established goalsDemonstrates ability to provide applicable care/service adjusting approaches to reflect developmental level of population served

Minimum Qualifications:

Registered Nurse or Registered DietitianCertified Diabetes Educator (CDE) BLS certification or certification within orientation period3-5 years of experience as a primary diabetes educator

Location: 14500 99th Ave N, Maple Grove, MN 55369

Hours:32 hours/week 0.8 FTE
