Join our compassionate team as a Certified Nursing Assistant (CNA) in the Transitional Care Unit, where you’ll provide essential, hands-on support to enhance the daily lives and well-being of our patients!
Shifts: Typically, 2p-10p & 10p-6a Transitional Care Unit (TCU): The Unit is a patient focused unit bridging the patient care journey from inpatient care services to their next long-term living environment. TCU provides a multidisciplinary approach to nursing care, aligning a long-term care experience in a hospital-based unit, leading to a well-rounded experience for our patients & team members alike!
Responsibilities: Provide direct resident care as directed by RN/LPN, including activities of daily living (e.g., bathing, feeding, dressing). Document resident care and safety checks according to procedures. Assist in unit tasks like passing meal trays, responding to call lights, and transporting residents. Report equipment malfunctions and ensure cleanliness of resident rooms and department areas. Perform routine tests (e.g., weights, vital signs) and assist with intake and output documentation. Communicate effectively and professionally with residents, staff, and families. Actively participate in feedback, learning, and unit improvement activities.
