Overview
Assists the Occupational Therapist in the clinical implementation of treatment programs by carrying out treatment plans as outlined in the OT evaluation, and assists the PACE CNY rehabilitation department with exercise/ADL/IADL and participant care related functions as needed.
Responsibilities
Job duties specific to this position:
+ Provides occupational therapy services to participants in accordance with OT Guide to Practice regulations, PACE policies and guidelines both at the PACE day centers and in the community.
+ Conducts effective ADL/IADL objective findings (in the day center and in participant’s homes) as allowed under the New York State Occupational Therapy Practice Act.
+ Establishes, monitors and modifies maintenance exercise and ADL programs/groups as needed.
+ Communicates with the interdisciplinary team, participant and/or family in order to provide a well-coordinated plan of care for all participants.
+ Provides emergency assistance for the facility as qualified and trained when required.
+ Provides documentation of participant treatment in accordance with policy/procedures.
+ Assists in orientation, supervision, and education of volunteers, students, and support personnel.
+ Participates in consultations and interdepartmental in-service education programs to enhance clinical and professional awareness of the practice of Occupational Therapy and its relation to other disciplines.
+ Assists in maintaining, organizing and coordinating appropriate inventory levels of departmental supplies and equipment.
+ Assists in maintaining a clean and orderly clinical area.
+ Actively participates in Quality Assurance initiatives as appropriate.
+ Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.
+ Ensures the highest quality of care by performing responsibilities according to the highest professional standards.
+ Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications
+ Graduate of AOTA approved school with an associate degree in Occupational Therapy.
+ Current NYS license in good standing or eligibility as a certified occupational therapy assistant.
+ Minimum of one year experience in geriatrics required.
+ Thorough knowledge of AOTA standards of practice.
+ Ability to carry out treatment plans and convey results to interdisciplinary team and families both verbally and in writing.
+ Ability to instruct participants in techniques for exercise, ADL’s, mobility, and personnel care.
+ Demonstrated skills in adaptation of activities that meet goals of treatment plans and are motivating to the patients.
+ Ability to educate participants, family and PACE personnel in ADL and adaptive techniques.
+ Proven ability to maintain accurate records of participant treatment.
+ Valid driver’s license and reliable vehicle.
+ Proficient in MS Office and able to learn new software rapidly.
An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
