Summary
JOB SUMMARY: The Case Manager directly partners with clients within various Center for Hope programs, providing support and guidance in reaching client goals. The Case Manager receives referrals from Center for Hope partners, including government and community agencies. This position is responsible for conducting client and family needs assessments, linking to indicated services or programs and providing a warm hand-off, helping clients to complete service and program applications, conducting home and community visits, conducting workshops and support groups, liaising between client and community resources, accompanying clients to court dates and other hearings, and conducting various forms of community outreach and education. The Case Manager is also responsible for other pertinent duties as assigned by the Program Manager.
REQUIREMENTS:
Bachelor's degree requred; working knowledge of theory and practice within a specialized field. Education Discipline: Social Work, Human Services, Family Studies, Family Science, Psychology, or other similar fields of study. Experience of 1-3 years required.Additional Information
