Job Location : Monday-Friday Onsite - Altamonte Springs, FL

The role you’ll contribute:

Under the direction of the Senior Manager or Manager of the Clinical Transformation team, the Change Coordinator will facilitate clinical pathway development, re-design process, data collection, and analysis. Responsible for the day-to-day implementation of clinical pathways to promote high priority quality, process, and safety initiatives throughout the institution. Collaborates with multi-disciplinary teams and diverse populations across the AdventHealth system to drive AdventHealth clinical transformation methodology. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

· Partners with clinical providers and operational staff to cultivate clinical and cultural change, leading the development of clinical workflow processes and data collection to facilitate the implementation of clinical pathways.

· Develops standardized metrics for clinical pathways, through gathering requirements, defining specifications, and establishing goals or benchmarks.

· Works with clinical pathway development teams to develop pathway components, assisting with project creation and analysis.

· Performs data entry of all data collected for assigned clinical pathway projects.

· Monitors and maintains accuracy and completeness of data in database under guidance of Analyst(s).

· Works with Management Information System to design and conduct electronic medical record (EMR) database searches, providing patient lists for clinical pathway enrollment purposes.

· Maintain data dictionary for each clinical pathway.

Qualifications

The expertise and experiences you’ll need to succeed:

KNOWLEDGE AND SKILLS REQUIRED:

· Strong written and verbal skills, with the ability to tailor messages to individuals and audiences throughout the institution

· Strong organization skills, with the ability to focus detailed concentrated effort to multiple clinical pathway projects and re-establish priority as necessary

· Ability to effectively respond to time sensitive issues and meet deadlines

· Initiative and ability to work independently while thriving in a setting requiring collaboration and teamwork for maximal efficiency and effectiveness

· Attention to detail that ensures follow through on all initiatives implemented and all projects undertaken

· Ability to conduct data entry and analysis in accordance with organizational and departmental goals

· Advanced knowledge of desktop applications, including MS Office and other web based applications for collecting/reporting data

· Basic knowledge of project management applications

· Excellent interpersonal and facilitation skills

· Awareness of performance improvement techniques

· Ability to lead quality improvement and/or process improvement activities

· Understanding of health information, diagnostic coding, and database management

· Analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principals

· Ability to lead and conduct basic secondary research (e.g., literature reviews) and advanced quantitative and qualitative analysis

EDUCATION AND EXPERIENCE REQUIRED:

· Bachelor’s degree in Business, Healthcare, Nursing, Allied Health or relative field OR

· Associate’s degree in Nursing or Allied Health or relative field AND three years of experience in healthcare environment, demonstrating analytical skills to resolve issues that may arise

EDUCATION AND EXPERIENCE PREFERRED:

· Two years of healthcare project management experience, data analytics experience and/or process improvement, quality improvement methodology/toolset
