**Responsibilities**
Under the supervision of the CHI Health at Home agency Director, the Hospice Chaplain is responsible for providing spiritual/bereavement care and counseling to Hospice patients, families, staff and volunteers.
**Essential Key Job Responsibilities**
_1. Documents assessment, planning and coordination of spiritual care of the patient/family:_
+ _Spiritual Assessment as documented in the electronic medical record._
+ _Patient/family care plan reflecting individualized goals and interventions._
+ _Spiritual assessment completed within five calendar days of admit._
_2. Meets Spiritual care needs of patient and family:_
+ _Visits made per frequency order and notes reflect ongoing assessment of spiritual needs._
+ _Increased visits/contacts as patient status declines._
+ _Provide spiritual/emotional support to patients and families._
+ _Coordinates spiritual care with community clergy as directed by the plan of care._
_3. Coordinates agency Bereavement Services:_
+ _Completes initial assessment within 4-6 weeks following request for bereavement follow-up_
+ _Develops bereavement plan of care; documents bereavement care in the electronic medical record._
+ _Provides grief/loss counseling to bereaved, as indicated in plan of care_
+ _Coordinates bereavement mailings_
+ _Conducts agency grief support groups_
+ _Coordinates hospice memorial events and community grief education events_
_4. Facilitates and/or coordinates in-services, presentations and special events for the Hospice program:_
+ _Assists in providing education for Hospice volunteers_
+ _Coordinates and assists in development of special events_
+ _Provides spiritual education to hospice team_
+ _Actively participates in committees working on Hospice related issues and needs._
**Qualifications**
Bachelor’s degree in Religious Studies or related field
Current driver’s license
Current auto insurance
BLS-CPR

**Pay Range**
$22.79 - $31.33 /hour
