Children/Family Programs
Centerstone’s children and family focused programs offer Case Management opportunities across community-based and school-based settings:
Community Action Treatment
Early Intervention Treatment
Family Intensive Treatment
Family Safety Interventions
School Based Prevention
Minimum Qualifications
Bachelor's degree required. Good organizational, time management, written and verbal communication skills. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries. Community-based program case managers must have a valid FL driver’s license.
Preferred Qualification:
Knowledge of community resources or ability to learn quickly. Experience working with individuals with mental health or substance use concerns. Enthusiastic about influencing positive change in our clients’ lives.
Special Skills, Knowledge and Abilities:
Competency with electronic medical records, computer systems, and documentation. Strong interpersonal communication skills in order to collaborate with team members and maintain professionalism. Accountable and self-directed with daily job duties.
Essential Functions:
Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor to ensure the smooth operation of the department.
Provides individual and group mental health and substance use disorder case management services.
Refers, advocates, and links client and family for additional services as identified.
Provides supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills.
Assist clients with developing natural support systems.
Transport clients in company vehicles to necessary appointments as needed.
Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
Conduct clinical behavioral health assessments.
Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
Act as liaison and communicate collaboratively with local law enforcement, and local stakeholders to maintain coordination of the treatment process.
Completes all required agency trainings to maintain 100% compliance.
Maintains departmental productivity expectations.
Additional training and/or certification may be required based on specific program guidelines.
Completes all other duties as assigned.
Work Environment/Physical Demands:
This position operates in community-based, office-based, and/or inpatient-based settings depending on program. Sits, stands, bends, lifts, drives, and moves intermittently during work hours. Is subject to frequent interruptions. Reasonable accommodations will be made as needed.
Position Type, Expected Hours of Work and Travel:
This is a full-time (40hours/week) position with a flexible work schedule to include early morning and evenings. On call rotation and weekend hours may be required.
