I. Major Responsibilities:

1. Implements program policies and procedures to maintain daily structure and consistency within the program and maintain compliance with all licensing standards.

2. Assesses clients basic physical, social, educational, legal and service needs and coordinates care with other inter-agency and community-based providers.

3. Works with multi-disciplinary treatment team to develop and update integrated treatment plans and recommendations including all service needs.

4. Develops appropriate comprehensive and appropriate aftercare plans while working with clients and members of multi-service team providers.

5. Conducts outreach and creates pathways for referrals. works closely with other providers to receive referrals; may conduct admissions assessments.

6. Assists clients in engaging next level of services.

7. Coordinates and provides transportation as needed.

8. Participates in multidisciplinary team to discuss client progress.

9. Makes referrals and assists clients with transition to behavioral treatment providers.

10. Leads skill development, educational or recreational group activities as assigned.

11. Attends program staff and clinical team meetings, supervision, trainings and all other required program meetings.

12. Documents treatment provided in EHR (Electronic Health Record), assessments, follow up interviews, case conferences, behavioral issues on the unit, coordination with other agencies or programs, referrals, clinical team treatment decisions, discharges and others.

2. Attends variety of meetings, conferences, seminars as required or directed.

3. Demonstrates use of Quality Improvement in daily operations.

4. Complies with all health and safety regulations and requirements.

5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.

6. Maintains, regular, reliable, and predictable attendance.

7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:

Required:

1. Bachelor’s degree.

2. Satisfactory CORI background check.

Preferred:

1. Bachelor’s degree in psychology or human services or a closely related field is preferred.

Experience/Skills:

Required:

1. Ability to develop a working knowledge of CHL’s service lines to triage individuals being assessed for services.

2. Excellent oral communication and computer skills.

3. Ability to multitask. Must possess time management skills with a keen sense of prioritization and follow through.

4. Requires personal and professional accountability, self-management, and initiative. Professional integrity and cultural competency.

5. Ability to use office equipment, including copy machines, computers, printers, telephones.

6. Ability to organize complex tasks and meet deadlines.

Preferred:

1. Prior experience in a behavioral health or social services setting.

2. Basic knowledge of CHL services and insurance requirements and reimbursement for these services.

3. Working knowledge of Electronic Health Records (EHR).

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

Position requires travel from base location to other facilities and patient residences with primary work being indoors in both patient care facilities and alternative patient care environments.
