Consider applying today. **Job Summary** The care coordinator supports patient care navigation of clinically complex patients in the clinic setting, managing care for the patients of the clinic to promote effective education, self-management support, and timely healthcare delivery. **Education and/or Experience** + Currently licensed as a Registered Nurse or LPN in South Dakota. + Previous experience in medical, surgical or pediatric nursing necessary. + Previous clinic experience preferred. + Leadership experience beneficial. **Licensure/Certification/Registration** + Currently licensed as a Registered Nurse or LPN in South Dakota.
