At M Health Fairview, we want to support your growth, honor your strengths and feel empowered to make a difference in the lives of others. **Responsibilities Job Description** The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, physician and/or authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines. Collects data for patient assessments: + Involves the patient, family, and other health team members in capturing relevant data + Document’s data collected in the medical record + Reports unusual data or changes in patient condition to the RN, or authorized practitioner in a timely manner Delivers care as delegated by the RN or physician to assigned patient population, or individual patient: + Follows the patient’s plan of care + Prioritizes work based on patient acuity, available resources, and patient preferences + Recognizes when different approaches may be needed based on age or cultural differences + Implements assigned aspects of the patient teaching plan + Documents care into medical records/EMR + Effectively communicates compliance and concerns to delegating RN/physician/authorized practitioner + Takes action to assure continuity of care for all assigned patients. + Responds appropriately to emergent situations Communicates effectively with other care team members: + Communicates verbal and written information in a clear, concise, accurate, and timely manner + Uses communication approaches appropriate to patient/support network needs **Qualifications** **Required** + Vocational/Technical Training or Associates Degree + Current LPN License **Preferred** + One to three years + Previous experience working in a clinic or in OBGYN + Current BLS Certification **We believe those that exceed in this role also exhibit these characteristics:** High standard of creative prioritization skills and priority setting.
