Apply today! *Responsibilities for Internal Candidates The Hospice Account Executive is responsible for executing the sales strategy to increase company market share through account development and educating the medical community on services provided while operating within a set budget. Build and maintain relationships with target referral sources to execute the bring care to more people (growth strategy) Implement, manage, and document consistent sales activities with multiple contacts in each referral source. Seek to better understand the needs of customers to provide customized solutions and earn new/continued referrals. Expand the healthcare community’s use of our services by supporting knowledge and awareness of our solutions. Serve as a liaison between our referral sources (community), our patients/families facing end of life care, and our agencies. Responsibilities Coordinates the referral and admission of patients to hospice from facilities Coordinates education and services with facility case managers, physicians and palliative care teams as appropriate Evaluates patient's eligibility and suitability upon request from the physician. Receives and writes referrals from the physician and sends to the appropriate hospice location. Acts as a liaison between the patient/caregiver and the physician and the hospice agency. Coordinates the care of the patient between the physician, facility and hospice agency and updates the hospice agency regarding changes through a conference note. Generates orders given by the physician and forwards to the hospice agency. Communicates to the hospice agency any orders or changes that the physician implements. Facilitates/orders DME equipment and sets up delivery Facilitates discharge ensuring that Hospice staff are available to admit and DME, medications and medical supplies are available Communicates with the hospice agency Executive Director regarding any complaints or concerns that the facility and/or physician has. Manages any referrals to hospice from the facility and coordinates communication regarding the referral/admit Responsible for executing the growth strategy of assigned facility accounts to increase company market share through relationship development and education. Serves as an educational resource to physicians, nurses and other practitioners within the facility as it relates to hospice. Attends the weekly Sales and Operations meetings of assigned agency Responsible for achievement of admission goals/expectations. Education and Experience Prior Hospice experience required. Must be a licensed RN in the state of practice Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
