Position Summary

The Clinical agent assists the medical personnel in clinical and administrative tasks and provide patient orientation, education, and support with clinical and administrative needs. The position will require some nights and weekends.

Essential Job Functions

Assist with daily patient flows in areas as needed. May initiate telehealth consultation by inviting and greeting patients before the actual visit. Communicate basic information about the nature of telehealth appointments to patients. Updates registration information in the electronic medical record. Assure special needs, such as physical accommodations or virtual translation services are provided. Answer questions and prepare patients for telehealth appointments. Record the medical history and additional personal information into EMR before the beginning of the visit. Assist with the setup of, and data collection from remote patient monitoring devices as needed. Assist patients with technical issues before and during telehealth appointments. Virtually notifies the care team when the patient is ready or if any support is needed. May assist with collecting patient-measure vital signs at the beginning of the virtual visit. Recommend telehealth services based on an understanding of patient conditions as established by the company. Educate patients about virtual visits. May provide follow-up care through virtual check-ins with patients under the direct supervision of a provider. Perform indirect patient care activities under the direction and supervision of a provider. Take appropriate actions to navigate patients and coordinate services. Maintain quality control and keep logs as needed according to the company's needs. Serve as liaison between physicians, patients, and other medical personnel. Coordinate patient care plans under the supervision of a provider. Document all clinical observation, information, updates, and patient education. Assist with processing refills based on company policies. Process and submit prior authorizations. Process insurance verification as needed. Assist with a transition to clinic process, prioritize clinic visits, and referrals to facilities. Perform EMR data entering. Assist with patient scheduling. Provide support to the patient remote monitoring team by monitoring patient's dashboards and proceeding as per protocol when an alarm is generated. Provide assistance by taking calls or replying to messages that need attention by the clinical staff. Able to rotate weekends, holidays, and shifts, and center location according to company needs. Perform other duties as assigned by supervisor. Supervisory Responsibilities

Physical Demands

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above the ambient noise level.

Employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders
