Clinical Assessment Manager, UAS (RN License required), Queens, NY

FlexStaff is seeking a Clinical Assessment Outreach Manager to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area for their patients in Queens, NY.

This is a permanent, full-time job. Schedule: Monday – Friday 8:30 AM – 5:30 PM.

In this role you will be responsible for intake clinical assessment, reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation.

An ideal candidate would have a nursing degree, a valid NYS RN License and minimum of one (1) year of experience working with a frail or elderly population and three to five (3-5) years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning (preferably within a managed care, home care, or community health organization). Proficiency in utilizing and analyzing UAS-NY and Lenavi.

Responsibilities include, but not limited to: 

Collaborates with intake, enrollment, and care teams for in-home or virtual assessments using UAS-NY, addressing PACE program benefits. Conducts UAS-NY in-home assessments for plan eligibility and care needs, focusing on Community Based Long Term Care Services. Reviews and updates past assessments, diagnoses, and medications in EMR, ensuring documentation accuracy and addressing significant changes. Educates and promotes the PACE program to potential participants and referral sources through various channels. Escalates assessment-related concerns to quality review for proper UAS-NY completion and issue resolution. Assesses home assistance needs using the PCAT Tool and provides service plan recommendations. Stays informed on CMS/PACE guidelines and responds to staff inquiries. Travels as required and works some weekends; supports Care Delivery with clinical insights and process improvements. Engages in quality improvement and reviews performance metrics for enhancement. Conducts 2-3 UAS-NY assessments daily, adhering to PACE competencies and contributing as an IDT member. Performs additional duties as needed.
