📝 Job Description:

As a Clinical Assessment Manager – Care Delivery, you'll play a crucial role in the clinical assessment process, including onboarding, reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation. You'll ensure compliance with Department of Health policies and serve as a valuable resource to our Intake, Enrollment, and Care Delivery teams.

🔍 Key Responsibilities:Collaborate with intake, enrollment, and care delivery teams for in-home or virtual assessment appointments.Conduct in-home assessments using the UAS-NY to determine eligibility for Community Based Long Term Care (CBLTC) Services.Evaluate and review previous assessments to ensure accurate documentation in the EMR.Promote the PACE program through presentations, lectures, and meetings.Educate potential and new PACE participants on plan benefits and services.Escalate questions or concerns to the quality review team for appropriate handling.Complete and/or reassess the PCAT Tasking Tool for home assistance needs.💼 Essential Skills:Active and unrestricted NYS Registered Nurse license.Minimum of one year of experience with a frail or elderly population.3-5 years of experience in MLTC, LTHHCP, MAP, PACE, or other managed care programs.5+ years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning.Proficiency in UAS-NY, Lenavi, CRM platforms, and Microsoft Office.Excellent communication and interpersonal skills.🌟 Additional Skills & Qualifications:Highly motivated and self-driven.Positive attitude with the ability to multi-task in a constantly changing environment.Flexibility to meet deadlines, including occasional weekend work.Customer service-oriented with strong clinical management skills.🏢 Work Environment:Professional office environment with standard office equipment.Schedule: 8:30 AM – 5:30 PM, 40 hours per week.Involves in-home or virtual assessments; some travel required.Must be medically cleared for communicable diseases and have up-to-date immunizations.Ready to make a difference? Apply now and join our team in delivering exceptional care and support to our community! Pay and Benefits

The pay range for this position is $110000.00 - $115000.00/yr.
