Clinical Care Assistant - High Touch Model

Position Summary

The Clinical Care Assistant assists in the registration, examination, treatment and overall management of patients and the data collected for patients. The Clinical Care assistant will perform duties under the direction of a physician or other provider in the medical centers for an employer sponsored program. The primary goal of the health centers is to provide health risk and chronic disease management to client company members based on predictive modeling and a baseline health assessment. This is a high touch position that requires individuals to be engaged with the patient population on a constant basis.

Essential Job Functions

Communicate effectively with employer group representatives to provide health center information. Ability to communicate actions back to supervisors and corporate staff on employer level communication habits. Interview patients, measure vital signs and record information on patient charts for employees and members of client contracted employer groups. Assist with daily patient flow in areas as needed. Provides direct patient care and performs indirect patient care activities and laboratory test, under the direction of a physician or other provider in the health center. Takes appropriate action to maintain a safe and aesthetic environment. Performs EKG, administer injections and assist in procedures/treatments as requested by physician or other provider in the health center. Performs lab work as per physician/provider orders and prepares instrumental equipment for sterilization. Adheres to all policies and procedures. Checks equipment and supplies prior to use for function, inspection and expiration date and reports malfunction of medical equipment and safety issues to supervisor. Maintains quality control (i.e., Glucometer, eyewash, refrigerator temperature and autoclave) and cleaning logs as needed for the health center. Complies with Infection Control and OSHA Regulations (biomedical wastes, handling and disposal) for the center as per regulations. Ability to process and handle confidential information with discretion. Degree of teamwork and cooperation with personnel from other departments. Maintain logging system for different procedures as required and enter nursing procedures and other information in the computer system. Check for accuracy in medical records and follow up obtaining missing information prior to patients’ appointment. Able to rotate weekends, holidays, and shifts and center location according to health center needs. Ability to adjust hours for early morning blood draws. Work on electronic medical record (EMR) entering patient information and maintaining confidentiality of patients and their records. Schedule and coordinate patient appointments and outside referrals using the employer preferred network of providers. Maintain site paperwork and send appropriate paperwork to employer. Maintain health center supplies and complete orders to maintain appropriate levels of supplies. Directly assist the onsite provider with day-to-day operations. Deliver other clinical procedures as directed by physicians and other providers. Work collaboratively with the team assigned to the employer group to deliver optimum care and coverage for contracted services. Participate in client sponsored trainings from time to time. Perform other duties as assigned by supervisor.
