Responsibilities Coordinate patient care across various disciplines to optimize treatment and outcomes. Develop and implement individualized care plans based on patient assessments and needs. Facilitate communication among patients, families, and healthcare providers to ensure continuity of care. Monitor patient progress and maintain up-to-date records in accordance with healthcare regulations. Educate patients and families about treatment options, medications, and care procedures to empower informed decision-making. Assist in scheduling appointments, referrals, and follow-up visits for patients. Participate in quality improvement initiatives aimed at enhancing patient care and operational efficiency. Requirements Bachelor's degree in nursing, public health, or a related field. 2 years of experience as a Registered Nurse in a home health setting. Current nursing license (if applicable) or relevant certification in healthcare coordination. Proven experience in a clinical setting, preferably in care coordination or management. Strong communication and interpersonal skills to effectively engage with diverse patient populations. Ability to work collaboratively in a multidisciplinary team environment. Proficient in electronic health record (EHR) systems and Microsoft Office Suite. Strong organizational and time management skills to handle multiple responsibilities and priorities.
