REQUIRES APPROVAL FROM SVP & COO, UT PHYSICIANS
Performs safe, effective clinical care for children/adolescent, adult, and older adult patients utilizing the nursing process. Demonstrates advanced nursing skills aimed at ensuring optimal patient care, effective communication and efficiency. Supports nursing research by participating in research activities, when appropriate. Assists in continuing education by assisting with teaching and orientation of staff and students. Collaborates with leadership to coordinate and implement performance improvement activities for the clinic. Provides outreach and education to hospitals and coordinates community based services to follow treatment plans.
Position Key Accountabilities:
1. Provides direct patient care approximately 50% of time
2. Effectively manages all aspects of the patient care process, and documents patient care and outcomes.
3. Participates in regularly scheduled team conferences, as required, and collaborates with related health care providers in treatment, planning and the provision of care.
4. Functions as the nursing liaison with physicians, other multidisciplinary team members, and community agencies/resources.
5. May promote treatment advances available through the clinicians and expand the referral base for the program through regular contact with community physicians and hospitals.
6. Serves as a resource for staff, to include the provision of information concerning evidenced based practice for procedures and other patient care developments.
7. Participates in the development of nursing policies and instructional materials for staff/students.
8. Acts as the lead nurse regarding management of supplies, equipment, and the patient care environment, to include notifying support departments of repairs needed for damaged property, equipment, etc.
9. Prepares and maintains reports concerning operations, as requested.
10. Assists in resolving interdepartmental / intradepartmental clinical problems and/or patient concerns, and leads identified performance improvement initiatives (chart reviews, PI initiatives, etc.).
11. Provides coaching and timely performance feedback to staff, as needed, and reports to Practice Manager/Supervising Physician for continued follow-up with staff.
12. Assists in the coordination of patient care conferences, etc., as indicated.
13. Insures timely completion of monitors / logs for quality control and performance improvement monitoring.
14. Manages other duties as assigned.
Certification/Skills:
Current nursing licensure in the State of Texas. Certification in area of specialty preferred.
Minimum Education:
Bachelor’s degree in nursing plus three years nursing experience, OR
Associates degree in nursing plus five years nursing experience, OR
Nursing diploma (3 year program) plus four years nursing experience.
Minimum Experience:
Three (3) years of clinical nursing experience required. Recent clinical experience in area of specialty preferred.
Physical Requirements:
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
