The Opportunity
FCA is seeking a Senior Case Manager for our Linkage Program. The linkage program provides short-term case management, counseling, education and linkage to long term care, community supports and services for those 55 years and older, residing in Nassau County that are facing behavioral health concerns, substance use disorders, challenges related to aging, disconnection from services, and struggling to access/connect to medical care. This is a great entry level opportunity for a recent Master's Level Graduate!
Responsibilities
+ Provide a blend of telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.
+ Maintain client case records, assessments, and statistics as required by the standards set by Family & Children's Association, New York State Office for Mental Health, & ARP (CSLFRF) grant guidelines.
+ Complete billable notes in an electronic health record system for counseling services.
+ Provide case management services to clients such as: assisting clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.
+ Accompany clients to appointments to provide support and advocacy, when needed, for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves in-person or via telehealth.
+ Participate in supervision with Program Coordinator and weekly Team Meetings; participate in agency trainings and workshops.
Qualifications
+ Experience with Geriatric population preferred, but not required.
+ Masters in Social Work or Mental Health Counseling required.
+ Valid New York State driver's license

Schedule:
+ Full time, 35 hours per week. Monday-Friday 9:00am-5:00pm
+ Opportunity for partial remote work schedule- technology provided
+ Salary: $28.57/hr
