Clinical Case Manager

Key Responsibilities: + Supervise the unskilled caregiving team, ensuring compliance with state regulations and agency standards. + Evaluate consumer needs and communicate required cHHA skillsets/experience to hiring team to build a robust care team. + Lead initiatives for caregiver training, competency assessments, and performance check-ins. + Develop and implement policies for case assignments and quality care management. + Coordinate client cases, maintain clinical records, and ensure high levels of consumer satisfaction. + Foster a safe environment by identifying areas for improvement and adhering to safety standards. + Maintain compliance with regulatory requirements to ensure we provide services with integrity. + Be available for on-call clinical care during emergencies or when needed. + Support the Director of Nursing (DON) with various assignments or tasks as needed. Requirements Qualifications: + Education: Graduated from an accredited nursing program with a valid D.C. RN license (BSN preferred). + Experience: Minimum of 1 year of supervisory RN and Home Care experience + Credentials: Current D.C. RN license, up-to-date Tuberculosis screening as per CDC guidelines, valid CPR certification, and reliable transportation. Skills Needed: + Ability to stay composed and professional in fast-paced environments. + Strong communication skills and interpersonal abilities. + Excellent time management for handling multiple tasks efficiently. + Commitment to confidentiality and respect for consumer privacy in accordance with HIPAA. + Leadership qualities that inspire a culture aligned with Open Systems Healthcare's core values.
